Sacroiliac Joint as Source of Back Pain: Pain Relief with Ultrasound Guided Cortisone Injections
Understanding Sacroiliac Joint Dysfunction: The sacroiliac joint, located at the base of the spine, connects the sacrum (triangular bone at the bottom of the spine) to the pelvis. It plays a crucial role in supporting the upper body and transmitting forces between the spine and lower extremities. When this joint becomes dysfunctional, either due to injury, inflammation, or degeneration, it can result in debilitating pain in the lower back, buttocks, and legs. This condition is known as Sacroiliac Joint Dysfunction or Sacroiliitis.
Diagnosis and Challenges: Diagnosing SIJD can be challenging as its symptoms often overlap with other back-related issues like herniated discs or lumbar spine disorders. However, a comprehensive clinical evaluation, including a thorough medical history, physical examination, and diagnostic imaging such as MRI or CT scans, can help pinpoint the exact cause of pain originating from the sacroiliac joint.
Treatment Options: While conservative approaches like physical therapy, NSAIDs, and corticosteroid injections can provide temporary relief, patients with persistent symptoms may require more targeted interventions. Ultrasound Guided Cortisone Injections have emerged as a promising treatment modality for SIJD.
Scientific Evidence and References: Numerous studies have demonstrated the effectiveness of Ultrasound Guided Cortisone Injections in managing SIJD. For instance, a systematic review and meta-analysis published in the European Spine Journal in 2020 concluded that corticosteroid injections under ultrasound guidance significantly improved pain and function in patients with sacroiliac joint pain compared to placebo or conventional blind injections (Vleeming et al., 2020). Another study by Maugars et al. (2014) reported sustained pain relief and functional improvement in patients with sacroiliitis following ultrasound-guided corticosteroid injections.
